What to Expect at Your First Appointment

Starting treatment can feel overwhelming. Our goal at your first visit is simple: understand your situation and create a safe, personalized plan, and get you started on buprenorphine immediately (i.e. the same day). 

Here’s what typically happens:

Before the Visit

Text/Call to Help Schedule a Visit

You’ll talk with a member of our outreach team to help you:

  • Verify that your insurance will cover the cost of your visit

  • Fill out medical consent forms over your phone/device (this gives the doctor permission to treat you)

  • Complete the Check-In form that tells us about your medical history and allergies

  • Upload your payment information securely over our encrypted platform

  • Click on the visit link to start your appointment at your scheduled time

During the Visit

1. We Listen to Your Story

You’ll talk with a medical provider about:

  • What substances you’re using (or trying to stop)

  • Your past experiences with withdrawal or treatment

  • Any history of precipitated withdrawal

  • Your physical and mental health

  • Your goals (stability, tapering eventually, pain control, etc.)

There is no judgment. This is medical care, not a test you can fail.

2. We Assess Safety

We review:

  • Current medications

  • Medical conditions

  • Pregnancy status (if applicable)

  • History of reactions to medications

This helps us choose the safest induction plan for you.

3. We Create Your Induction Plan

Your starting plan depends on your situation:

  • Kratom / prescription opioids → Often a standard or slightly modified start

  • Fentanyl or heroin → May use a macro-induction or carefully timed start

  • History of precipitated withdrawal → More customized strategy

  • Chronic pain → Dosing may be split or adjusted

You’ll receive clear instructions through an app that will walk you step-by-step through:

  • When to stop other opioids

  • When to take your first dose

  • How much to take

  • What to do if symptoms come up

4. We Prescribe Medications during the visit

Your medications are sent during the visit:

  • Opioid withdrawal is scary, so we understand you want reassurance that you will get relief through medications

  • Medications are e-prescribed directly and immediately to your pharmacy before you leave your visit

  • We use the pharmacy of your choice

  • You will coordinate with your pharmacy for pick up

You are not expected to wait around and wonder if your medications were sent after the visit.

After the Visit

1. Start Your Medications ASAP

Once you sign out of your visit:

  • You call your pharmacy to coordinate pickup

  • If you run into pharmacy issues you can text/call our team

  • Start your medications as prescribed so you can feel better as soon as possible

2. Follow-Up Visit

Once you’re stable, we shift focus toward:

  • Relapse Prevention

  • Balancing life, work, and family

  • Support groups (if you want it)

  • Long-term options like maintenance, tapering, or Sublocade

The first visit is about getting you safely started. Everything after that builds on stability.
